GETTING STARTED

Send an email regarding interest in services. An intake form will be sent to you to gather information about your child, communication goals and concerns, and their interests. You may also fill out the form located in the Contact Tab.

Example:

Hello! I am writing to express interest in therapy services for my son/daughter. Can you please share the intake form?​

A phone consultation will be held following review of the intake form. We will create a plan and schedule  for your child based on their information.

Complete all paperwork to begin services:

• Client Intake Form

• HIPAA Privary Note & Payment Policy

• Service Consent Form

• Communicate Release (if needed)

Paperwork and treatment plan will be reviewed. Once established, treatment can begin!
